Course XII - Root Cause Analysis - 2 Days
Introduction
Attendees in this two-day Root cause analysis (RCA) will learn problem solving methods focused on identifying the root causes of problems or events. The Root Cause Analysis (RCA) process is structured on the belief that problems are solved by trying to correct or eliminate root causes, as opposed to simply applying a band aid by addressing the immediately obvious symptoms. By focusing corrective measures at the identified root causes, the likelihood of problem recurrence should be minimized. Identifying the apparent root cause does not always guarantee the problem is completely resolved in a single intervention, so the RCA process is an iterative process of continuous improvement. With experience in the RCA process, it becomes a proactive methodology in the ability to forecast event before they occur. Attendees will learn the concepts and procedures for conducting Root Cause Analysis activities using many different tools, focused on the four M’s (methods, machinery, manpower, and materials) and E (the environment).

Organizational personnel who are accountable for implementing process and equipment change, or maintaining equipment, to include Engineers, Maintenance Managers and Supervisors, Operations managers and Supervisors, and Maintenance or Reliability Engineers.
